Location: The Salvation Army - Owens Campus

Status: Fulltime Non-Exempt

RESPONSIBILITIES

  • Provide mentorship to VA Contract staff and oversee day to day operations
  • Assure VA reporting compliance
  • Facilitate the intake and discharge process.
  • Assist with the facilitation of staff meetings and conduct file review
  • Provide direct case management services to homeless veterans in Contract (64), CIP (20), and overflow in Personal Safety Dorm, as needed.
  • Complete accurate and timely data entry into the HMIS system.
  • Coordinate care with VA personnel and others on behalf of participants for eligibility documentation,
  • Mental and physical health services.
  • Maintain client databases and referrals for services and generate reports.
  • Maintain, record, and report progression of veterans through program participation.
  • Oversee compliance with all facility rules.
  • Assist veterans to identify mainstream needs and facilitate the process with appropriate service providers to meet those needs.
  • Assist veterans to alleviate employment barriers.
  • Oversee the intake, distribution, storage and destruction of medication logs for all on-campus Veteran program participants.
  • Work closely with VA pharmacy assuring Veteran participants make appointments and place refill orders when medications are low
  • Maintain Veteran Services vehicle log book, including gas card responsibility and maintenance/service records.
  • Attend all scheduled team meetings and trainings.
  • Other duties and special projects as assigned.

QUALIFICATIONS

  • Bachelor's degree in Social Work or related discipline. A minimum of three years related employment experience may be substituted for degree requirements.
  • Experience working with homeless and/or at-risk veterans preferred.
  • Strong oral and written communication skills.
  • Proficiency in Microsoft Office Software (Word, Excel, etc.)
  • Ability to communicate and work effectively with a diverse group of residents, staff and community partners.
  • Must have a valid Driver License, clean driving record.
  • Veteran Status Preferred.

Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to focus. The employee must have the ability to operate a telephone, desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
